POLAR EXPRESS PARENT DATE NIGHT-OUT REGISTRATION FORM

We are SO excited & cannot wait for Polar Express Night! Parents, you can do some Christmas shopping and go on a date to see the Christmas lights while we will love and nurture your littles and watch The Polar Express !

Date: Friday, December 12th

Ages: 2 years - 10 years old

Place: Kingdom Kids Daycare - 712 S Slaughter Avenue, Sundown, TX - 229-3640

Time: 6:30 pm -10:00 pm*

Online registration form below, or forms may be picked up at Kingdom Kids Daycare or Sundown Church

Spots are limited, so we encourage you to sign kids up early!

POLAR EXPRESS NIGHT FOR THE KIDDOS ACTIVITIES:

• Polar Express Movie

• cocoa

• chocolate milk + regular milk

• snacks

• songs

• and praising Jesus !

• …there may even be a special guest to punch their ticket …. (Mr. Conductor)

DETAILS:

• Kids are welcome to wear robes &/or warm, long pajamas like the kids in the movie

• Kids need to have eaten supper prior to drop-off, please

• Kids need to wear regular shoes & bring a warm coat as they may go outside to play depending on the weather

• We kindly ask prompt 10:00 PM* pick-up or earlier
